IBS and low-carb eating: why the belly settles down
One in ten people are diagnosed with IBS, women twice as often, and after all the tests they are told to “live with it.” Meanwhile, the best-supported approach for IBS is a diet low in fermentable carbohydrates, and the method overlaps with it by two-thirds: sugar, flour, legumes, apples, pears, honey, and sweeteners disappear on their own. Section — heartburn, gallbladder, sensitive gut.
What IBS is and what feeds it
Bloating, pain, constipation, or diarrhea — or all of them in turn — with clear tests and a normal colonoscopy. The mechanism: the gut is more sensitive to stretching, and it is stretched by gas from bacteria that ferment undigested carbohydrates. The list of these carbohydrates is known: lactose, fructose, fructans in wheat, onions, and garlic, galactans in legumes, sugar alcohols — sorbitol, mannitol, xylitol. Remove them — less gas, less pain.
Foods in the method and IBS
| Food | On the method | With IBS | What to do |
|---|---|---|---|
| Sugar, flour, legumes, apples | no | no | already removed |
| Onions, garlic | yes | often trigger symptoms | remove for a month, reintroduce one at a time; the green part of the onion is allowed |
| Cabbage, broccoli, cauliflower | yes | raw — trigger symptoms | cooked only, in small portions |
| Avocado | yes | more than half triggers symptoms | half per meal |
| Mushrooms | yes | often trigger symptoms | limit, test |
| Erythritol, xylitol, sorbitol | “keto sweets” | strongly trigger symptoms | remove completely |
| Milk, soft cheeses | limited | lactose triggers symptoms | hard cheeses and butter are allowed |
| Meat, fish, eggs, butter | yes | neutral | the basis of the plate |
| Zucchini, cucumber, spinach, tomato | yes | neutral | the vegetable base |
How to build a plate with IBS
For the first month, have a “calm” plate: meat, fish, eggs, zucchini, cucumber, spinach, tomato, lettuce, olive oil, hard cheese, nuts 30 g. No onions, garlic, raw cabbage, mushrooms, or keto desserts. Three meals and a fasting window — the pause gives the gut time to “clean up,” and with IBS this works no worse than food. Then reintroduce one food every three days and keep notes. Usually after two months, the list of restrictions shrinks to two or three items. About fasting and the gut — digestion and fasting.
What else matters
Stress and sleep — with IBS, the gut and brain are directly connected, and lack of sleep can cause a flare without any food at all. Walk every day. Peppermint capsules are one of the few treatments with evidence for pain. Probiotics work for some people; choose one strain for 4 weeks, not a “complex.” And sauerkraut — starting with a teaspoon, not a plateful.
Red flags — not IBS, see a doctor
- Blood in the stool
- Unintentional weight loss
- Pain that wakes you at night
- Onset of symptoms after 50
- Anemia on a test
- Fever, vomiting
Frequently asked questions
IBS with constipation — won’t the method make it worse?
It may happen in the first few days if you are low on water and magnesium. Water with salt, magnesium citrate, psyllium — and the constipation-predominant type usually goes away along with the bloating.
Full FODMAP protocol or the method?
The method already removes two-thirds of FODMAPs. Add eliminating onions, garlic, raw cabbage, and sweeteners — and it is almost a complete protocol without counting.
Without onions forever?
No. After 1-2 months of a calm plate, reintroduce it and see. Many people tolerate cooked onions normally.
