Reflux is fueled by three things: sugar and flour, which ferment and create pressure from below, large portions, and a late dinner. On a low-carb diet, heartburn goes away in most people within two to three weeks — this is one of the most common «side-effect» improvements. For some, it gets worse during the first days: coffee on an empty stomach, fat without food, or a portion twice as large as needed. The rule: smaller portions, dinner before 19:00, coffee with food rather than before it, and three hours between dinner and sleep. Bile is needed to digest fat. On a low-fat diet, the gallbladder becomes sluggish, bile stagnates, and this — not fat — causes stones to grow. A sudden switch to 70% fat makes it work at full capacity immediately: heaviness on the right, bitterness, and nausea after fatty food. The solution is to increase fats gradually, have three meals instead of six, use bitter foods and lemon before meals, and stay active. IBS is a diagnosis of exclusion: bloating, pain, constipation, or diarrhea when everything has been examined and nothing has been found. It is fueled by fermentable carbohydrates — sugar, flour, onions, garlic, legumes, apples, and sweeteners. A low-carb diet automatically removes most of this list, and IBS subsides within a month for many people. Meat, fish, and eggs require acid. After 45, when taking omeprazole, and with gastritis, there is less acid, and protein sits heavily in the stomach, causing belching and bloating an hour later. What helps: chew longer, eat smaller portions, drink lemon water or water with apple cider vinegar before meals, and use bitter foods — arugula and chicory.
