Metabolic health: five numbers that matter more than weight
In 2018, American scientists calculated how many adults could be called metabolically healthy based on five simple indicators — waist, blood sugar, blood pressure, triglycerides, and “good” cholesterol. The result was 12%. Eight out of ten people with a normal weight also failed. Metabolic health is not about weight, but about how the body handles energy: whether it can keep blood sugar steady, switch from carbohydrates to fat, and avoid storing everything in the abdomen and liver. Five numbers instead of one on the scales
Metabolic syndrome is diagnosed when three out of five criteria are present: waist circumference of 80 cm or more in women, triglycerides of 1,7 mmol/l or more, HDL below 1,3, blood pressure of 130/85 or more, and fasting glucose of 5,6 or more. None of them is weight. A thin woman with a waist of 84, triglycerides of 2,0, and blood sugar of 5,8 is metabolically ill with a BMI of 22; a larger woman with a waist of 79 and normal test results is healthy. Blood sugar: swings you cannot see
A bun for breakfast — blood sugar reaches 8-9 an hour later, insulin is released in excess, and blood sugar falls below its starting level by 11:00 — hunger, weakness, “my brain does not work,” and the next bun. These are swings, and each amplitude is stress for the blood vessels and pancreas. The liver: the organ that responds to sugar first
Fructose — half of sugar, and all the fructose in honey and juices — is processed only by the liver, and it turns the excess into fat, which it stores in itself. Fatty liver affects one in four adults, most without symptoms, discovered “by chance” on an ultrasound. It is not alcohol — it is sugar, and it is the first step toward insulin resistance and diabetes. The good news is that the liver is the fastest-recovering organ, and without sugar and fructose, fat leaves it within 2-4 months; ALT falls before weight does.

Cholesterol: the number people read incorrectly
Total cholesterol says almost nothing. Triglycerides and HDL do: a ratio below 2 means there are few small, dense LDL particles and risk is low; above 3-4 means the opposite, regardless of total cholesterol. On a low-carb diet, triglycerides fall and HDL rises — both moving in the right direction; LDL rises in some people, especially those who are lean, and this requires separate analysis, not panic and not ignoring it.
