Heartburn and reflux: why they go away without sugar and when they get worse
Heartburn is one of the first things to go away on the method: studies show a reduction in reflux symptoms as early as one week on a low-carbohydrate diet, often before stopping medication. But for some people it flares up instead in the first few days—and always for the same reasons. Section — heartburn, gallbladder, sensitive gut.
Why reflux thrives on sugar and flour
Carbohydrates that have not been absorbed ferment in the stomach and small intestine, gas pushes upward, and the sphincter between the esophagus and stomach opens. Large portions do the same mechanically. A late dinner—when lying down, acid moves upward on its own. Remove sugar and flour and reduce portions—the pressure from below disappears, and the sphincter holds. This works for most people within two to three weeks.
Why it may get worse in the first few days
Coffee on an empty stomach means acid on an empty stomach. Fat without food—coffee with butter, a spoonful of oil “for satiety”—relaxes the sphincter. A portion meant to “fill up for 16 hours” stretches the stomach. Mint and chocolate also relax the sphincter. Lying down or bending over immediately after eating. All five are first-week habits, and all five can be removed.
A three-week protocol
| What | How | Why |
|---|---|---|
| Portions | one-third smaller, three meals | do not stretch the stomach |
| Dinner | by 19:00, 3 hours before bedtime | acid does not move upward while lying down |
| Coffee | only after eating, not on an empty stomach | acid on an empty stomach |
| Fat | as part of a meal, not separately | fat on an empty stomach relaxes the sphincter |
| After eating | walk for 15–20 min, do not lie down | gravity is on your side |
| Head of the bed | raise by 10–15 cm | nighttime reflux |
Acidity after 45 and omeprazole
A paradox: heartburn after 45 is often caused by too little acid, not too much. When there is too little acid, protein sits and ferments, creates pressure, and the esophagus is burned by what rises up. Omeprazole relieves the symptom and worsens the cause. You must not stop it on your own—the rebound effect causes a flare—but it is worth discussing a dose reduction with your doctor while following a low-carbohydrate diet: the need often disappears. More details — stomach acidity.
See a doctor, not your plate
- Heartburn lasting more than 3 weeks on the protocol
- Chest pain radiating to the arm or jaw—rule out the heart
- Difficulty swallowing, food “gets stuck”
- Vomiting blood, black stool
- Unplanned weight loss
- Heartburn for the first time after 50
Frequently asked questions
Baking soda for heartburn—is it okay?
Once—it will relieve it. Regularly—no: sodium in doses that disrupt the balance and an acid rebound afterward.
Fasting and heartburn?
For some people, the stomach produces acid on schedule during the first few weeks. Water with a pinch of salt, not coffee; make the eating window one hour shorter for two weeks.
Does apple cider vinegar help or harm?
With low acidity, it helps digest protein. With erosions and active reflux, it burns. Try a teaspoon in a glass of water before eating; if it burns, this tool is not for you.
