How to read a lipid panel: a five-step process without panic
Total cholesterol is the last thing to look at. Order: triglycerides, HDL, ratio, LDL in context, ApoB when in doubt. Ranges, pitfalls, and what to bring to the doctor.
“You eat so much fat — what about your cholesterol?” Every woman on the method hears this question. The answer is in the test results, and it is more interesting than fear: triglycerides fall by a third, “good” cholesterol rises, and blood pressure drops — three heart markers moving in the right direction in three months. And LDL, the so-called “bad” cholesterol, rises in some people; this is where you need not fear, but know how to read the report. The main page for the topic is metabolic health.

Triglycerides — fat in the blood that the liver makes from sugar and fructose — fall first and the most. HDL — the “good” cholesterol that removes cholesterol from the blood vessels — rises with dietary fats, especially saturated fats and omega-3s. Total cholesterol may rise — and that means nothing without a breakdown. LDL is unchanged or slightly lower in most people, but rises noticeably in 20-30%, more often in lean people. Analysis — cholesterol on a low-carb diet.
Divide triglycerides by HDL (in mmol/l): below 1 — excellent, 1-2 — good, above 3 — poor. Why this matters more than LDL: the ratio reflects LDL particle size — with low triglycerides and high HDL, particles are large and “fluffy”; they hardly penetrate the vessel wall. With high triglycerides, they are small and dense, and these are the ones that form plaques. The same LDL number with a different ratio means a different risk. Analysis — triglycerides and HDL.
| Marker | Reference range | What it shows | On the method |
|---|---|---|---|
| Triglycerides | below 1,7, preferably below 1,0 | sugar and fructose in food, insulin | fall |
| HDL | above 1,3 in women | vascular protection | rises |
| LDL | below 3,0 by the standard | context matters more than the number | rises in some people |
| Total | up to 5,2 | the sum of everything, not very informative | may rise due to HDL |
| TG/HDL | below 2 | particle size, actual risk | improves in almost everyone |
| ApoB | below 0,9 g/l | number of atherogenic particles — more accurate than LDL | depending on the situation |
Neither panic nor ignoring it. First, look at triglycerides and HDL: if both are excellent, the particles are large, and most cardiologists consider this a different picture from high LDL with high triglycerides. Second, check ApoB and, if available, particle size. Third, consider family history, blood pressure, blood sugar, and inflammation. Fourth, you can reduce saturated fats in favor of olive oil, avocado, and fish while keeping a low-carb plate: LDL usually falls without losing the other benefits. Make the decision with a doctor based on the full picture. How to read the report — how to read a lipid panel.
Insulin makes the kidneys retain sodium and water — blood pressure rises. When insulin falls, water is released and blood pressure drops by 5-10 mm in the first weeks, and in some people by 15-20 over three months. If you take medication, monitor at home from day one: doses may become excessive, and dizziness when standing is a signal to talk to your doctor, not to stop eating salt. Salt is needed on the method. Analysis — blood pressure on a low-carb diet.
Before menopause, estrogen protects the blood vessels and keeps HDL high, so the risk of a heart attack in women is lower than in men. After menopause, this protection disappears, and over 10 years the risk becomes similar. At the same time, symptoms in women are different: not “chest pain,” but fatigue, shortness of breath, nausea, and jaw pain — and they are more often overlooked. Therefore: a lipid panel, blood pressure, and waist measurement after 45 — once a year, not “when something starts hurting.” The three markers the method improves fastest — triglycerides, HDL, and blood pressure — are exactly the ones that deteriorate first in women after menopause. About perimenopause — after 40: perimenopause.
Dietary cholesterol has almost no effect on blood cholesterol for most people: the liver regulates it itself. Eggs are part of the method’s plate.
After 3 months, not earlier: during the first weeks, the numbers “fluctuate.” Fast for 12 hours, after a normal week, not after holidays.
Compatible. The decision about statins belongs to the doctor, based on the full picture and risk. The method improves triglycerides, HDL, and blood pressure independently of them.
During the first 6–8 weeks, the markers readjust — the numbers “fluctuate.” An early test can cause unnecessary alarm.
Not after the holidays, not after vacation, not after three days of fasting. Eat as you normally do.
Triglycerides are 1.5–2 times higher after eating. Water is allowed, coffee is not.
Total cholesterol, LDL, HDL, triglycerides — the minimum. ApoB if LDL is ambiguous. Lp(a) — once in a lifetime.
Triglycerides, HDL, ratio, LDL in context, total cholesterol last. With this order, see your doctor.

Insulin retains sodium and water — blood pressure rises. Insulin drops — water leaves, and blood pressure decreases by 5-15 over several weeks. What this means for people taking medication, and why salt is not the enemy.
Triglycerides down by a third, HDL up, blood pressure down — three heart markers moving in the right direction. And LDL, which rises in 20-30%. How to read this without panic or ignoring it.
Divide triglycerides by HDL. Less than 1 — excellent, more than 3 — poor, regardless of total cholesterol. Why this ratio shows particle size and actual risk, and what drives it.
