Belly After 40: Why It Appears and How It Goes Away
The weight is the same, but your jeans no longer fit around the waist — this is the most common complaint after 40. The fat hasn’t increased; it has moved: from the hips to the belly, and some of it has shifted inside, around the organs. It’s not about eating more. It’s about estrogen, insulin, and cortisol, and it won’t be eliminated by starving yourself or doing ab exercises. Section — perimenopause and after 45.
Three reasons
The first is estrogen. While there is plenty of it, fat goes to the hips and buttocks; when it drops, the body switches to a “male” pattern and stores fat around the waist. The second is insulin: sensitivity to it decreases along with estrogen, and carbohydrates that used to be processed now go to the belly. Read more — insulin resistance in women. The third is cortisol: stress and poor sleep keep it high, while cortisol stores fat specifically on the belly and takes muscle away. And on top of that — muscle mass is lost, energy expenditure drops, but food intake remains the same.

Why abs and cardio don't work
Spot fat reduction does not exist: one hundred crunches strengthen the muscle under the fat, but they do not affect the fat above it. An hour of cardio every day raises cortisol and appetite — for women over 40, this often produces the opposite result. A calorie deficit of 1200 kcal takes away muscle and lowers energy expenditure. Everything that “should work” works against you. What works is what lowers insulin and cortisol and restores muscle.
What gets rid of belly fat
Low-carb plate without snacks
Cut out sugar, flour, and starch. Three meals a day. Insulin drops, and visceral fat goes first—this is visible at the waist earlier than on the scales.
14/10-16/8 window
Fasting periods push insulin down further. Dinner by 19. Longer than 16 hours—no: cortisol.
Strength training 2-3 times a week
Muscles restore energy expenditure and take up glucose. Cardio—walking, not intervals every day.
7-8 hours of sleep
Without it, cortisol will not drop, and the belly will not go away. What interferes with sleep after 45—in the article about hot flashes and sleep.
Protein 1,6 g/kg
Muscles are built from it. Without protein, strength training does not work.
How to measure
Not with scales. Use a measuring tape at navel level, in the morning, before eating, once every two weeks. Minus 1 cm every two weeks is a good pace, and it often happens with a stable weight because muscles are increasing. Threshold: 80 cm — a health risk, 88 — high. A side-view photo once a month will show more than any number. The overall context of weight at this age is nutrition during perimenopause and the method for women 40+.
Frequently asked questions
How long until the belly goes away?
The first centimeters — in 3-4 weeks of low-carb eating without snacks. A noticeable result — 3 months. A complete result — six months with strength training and sleep.
My belly is hard and protruding, but I’m thin. Is it fat?
More often, it is visceral fat plus a weak pelvic floor or diastasis after childbirth. A tape measure and ultrasound will provide the answer. With diastasis, start with pelvic floor work, then everything else.
Do body wraps, massage, and waist belts help?
No. Visceral fat cannot be reached from the outside. Only insulin, cortisol, muscles, and sleep.
