Triglycerides and HDL: one ratio instead of total cholesterol
Two women with LDL of 3.8. The first has triglycerides of 0.7 and HDL of 1.9. The second — 2.4 and 1.0. The same “bad cholesterol,” but the heart attack risk differs several-fold — because different particles stand behind that one LDL number: large and harmless in the first woman, small and dense in the second. What the ratio shows
Triglycerides rise from sugar, fructose, and insulin resistance; HDL falls from them as well. When triglycerides are high, the liver assembles LDL into small, dense particles — they penetrate the vessel wall more easily and oxidize. When they are low, the particles are large, “fluffy,” and their atherogenicity is significantly lower. The TG/HDL ratio is an indirect but reliable marker of particle size and insulin resistance, and studies show that it predicts cardiac events better than LDL.

What drives each one
Triglycerides down: zero sugar and fructose — juices, honey, and fruit in quantity; less flour and starch; no alcohol; omega-3 2-3 g; a fasting window. This is the most responsive marker — down by a third in three months. HDL up: dietary fats — olive oil, avocado, fish, eggs, cheese; strength training and walking; quitting smoking; and time — HDL rises more slowly, over 3-6 months. Mistakes
- Looking only at total cholesterol and LDL

Triglycerides 2.5 — what comes first?
Juices, honey, sugar, alcohol — zero. After 3 months, it is usually below 1.5. This is the fastest marker.
Low HDL with normal triglycerides?

Does this replace ApoB?
