Walking with joint pain: cartilage lives through movement
Cartilage is the only tissue in a joint without blood vessels. Blood does not reach it; it is nourished by synovial fluid, which is pumped through it when it compresses and relaxes — that is, with each step. When you lie down, cartilage starves. When you walk, it eats. That is why, for osteoarthritis, walking comes first in clinical guidelines, before medication. Section — training with joint pain.
How much
As much as does not cause increased pain the next morning. Start with 10-15 minutes a day, adding 5 minutes per week; the goal is 30-45 minutes daily or 7-10 thousand steps. Two 15-minute walks are better than one 30-minute walk if it hurts after 20 minutes. The pace should allow you to talk. There is no need to hurry: cartilage responds to consistency, not records. About steps in general — walking and daily steps.
Where, what to wear, and how
| What | Better | Worse | Why |
|---|---|---|---|
| Surface | level asphalt, dirt, treadmill | cobblestones, gravel, descents | unevenness — impact and twisting |
| Footwear | with cushioning, arch support, laces | flat soles, flip-flops, heels | every step — an impact on the knee |
| Stairs | up — allowed, down — take the elevator | going down with pain | downward — up to 7 times your body weight on the knee |
| Pace | conversational | fast enough to cause pain | the load increases with pace |
| Poles | Nordic walking with knee and hip pain | — | reduce the load by 20-30% |
| Time | in the morning or during the day | in the evening if there is swelling | swelling by evening — a sign of overdoing it |
When walking is harmful
Acute swelling and a hot joint — first reduce the inflammation, then walk. If pain increases with every step instead of easing after warming up — stop. If you have flat feet and no support for the foot and ankle — get suitable footwear and insoles first. With pronounced hip osteoarthritis, use poles or a bicycle instead of long walks. If you weigh over 100 kg, start with 5 minutes and cycling, and increase the load more slowly. About knees and weight — knees and excess weight.
Walking and training — together
Walking nourishes cartilage but does not build muscle. Muscle is built through strength training two or three times a week — training with knee pain. Together they provide what neither can provide alone: living cartilage, a supported joint, and weight loss. Plus, walking after a meal lowers blood sugar and aids digestion — an added bonus.
Frequently asked questions
Treadmill or outdoors?
A treadmill is softer and more even — better for starting. Outdoors offers variety and fresh air once the pain has gone.
Bicycle instead of walking?
With severe knee and hip pain — yes, it reduces the load. But walking is movement you live with, and you need to return to it.
It hurts after walking — does that mean it is harmful?
Mild fatigue is normal. Pain that is worse tomorrow than it was yesterday means you overdid it: reduce the time by 5-10 minutes, then build it up again.
